Care Assistant - Specialist Care Pathway

Abbots Care Ltd. are the Care Company and Employer of choice within Hertfordshire, UK. We are excited to be working alongside the local hospital discharge team and Specialist health services at Hertfordshire County Council.

We are currently looking to recruit experienced Care Assistants to join our established Care Specialist team covering arears such as Watford and Three Rivers. We offer average guaranteed earnings from £19,500 (based on a 39 hour week) + 30p per mile + 28 days Holiday pro rata. You will play a fundamental role in supporting and enabling our service users discharged from NHS hospitals to recover and rehabilitate at home.

This is an exciting and crucial role to ensure that intensive enablement is delivered with a view to reduced and prevent hospital admissions. You can really make a positive difference in this role and play a key part in ensuring service users wellbeing and recovery.

You can expect to be;

Involved in service users’ reviews, developing persons centred and individual support plans.

Deliver outcomes that best meet the service users need.

Using a person-centre approach, encourage and provide solutions to assist people to manage their lives independently for as long as possible.

Requirements

Caring and a good communicator Experienced Care Assistant and prepared to work towards a diploma in social care Full and valid driving licence and have access to your own vehicle Approachable and confident Hands on, flexible and organised Understand the needs of person focused services Able to deliver outcomes-based services Willing to make a real difference Benefits

Guaranteed 39 hours per week contract.

Salary from £19,500 per annum

Paid mileage - £0.40p per mile between Service User visits

28 days holiday, pro rata
